    A Community Clinic

    Individuals must reside in Montour, Northumberland, Snyder, or Union Counties and must be under 65 years old. Individuals are required to apply for medical assistance and bring proof of a denial letter, proof of household income, and a photo identification card.

    A Community Clinic provides primary care, chronic care, referrals, and prescription services for individuals without health insurance. Please call to schedule an appointment. Please note that individuals must travel to Northumberland County to receives services.

    The Columbia-Montour Aging Office serves older adults in Columbia and Montour Counties. The Columbia-Montour Aging Office provides care-managed services, home health services, nursing home transition services, in-home services, adult day care services, access to senior centers, meals, health insurance counseling, legal assistance, free transportation services, and more. Please note that an individual may have to travel to Columbia County to receive certain services.

    PA Law Project

    The PA Health Law Project works to ensure everyone has access to health care. Outside of advocating for the right to health care, the Law Project provides free legal services to any low-income resident of Pennsylvania having difficulty accessing publicly funded health care, whose health care has been terminated or reduced, or who have been denied Medicare (or similar health care programming). Additionally, the Law Project helps eligible residents apply for Medicare, responds to issues of denial of service (i.e. for prescriptions), and anyone struggling with their health care costs. The only requirement is income related, otherwise, the program is nondiscriminatory - there is a helpline which can be utilized for base information by anyone, regardless of income.     COMPASS is an online application for Pennsylvanians to apply for health and human services programs. The application includes Health Care benefits such as Medical Assistance, CHIP, and Medicaid for Former Foster Care Youth. The application also includes Home and Community Based Services Referrals such as Early Intervention Services, Intellectual Disability Services, and Autism Services.
