Adams County Office for Aging

The Adams County Office for Aging is a private non-profit agency dedicated to providing assistance to and advocacy for people age 60 and above within Adams County. Our mission is to assist people in maintaining independence and dignity within their own homes and communities. We offer a variety of services to support our age 60 and over population including; adult day care, senior centers, Lifeline systems, medical equipment loan, volunteer opportunities, tax preparation assistance, Medicare counseling, transportation, property tax and rent rebate assistance, protective services, legal services, and a variety of health and educational programs. For in-home care, individual can self-refer or a medical professional can refer as well. As for the other services, interested person should just call ahead and let them know they are coming.

Allegheny County Jail Collaborative

The Allegheny Jail Collaborative is a program committed to reducing recidivism by both preparing inmates for reentry while incarcerated and helping them be successful people upon reentry into society. The program provides services for both inmates and returning citizens. For inmates, the program's services include behavioral therapy, drug and alcohol rehabilitation, classes on GED track, pre-vocational classes and vocational training, and family support. Reentry services include cognitive and behavioral therapy, a specialized Batterer Intervention Program, drug and alcohol assessment, housing services, GED, job placement and training, and transportation services.

The Beaver County Office on Aging is committed to identifying the diverse needs of our aging population, providing quality services, preserving the dignity of the individual and advocating for the older person's rights. The Center for Community Action (CCA) provides a variety of services and programs to low-income community residents. CCA provides assistance with accessing Medical Assistance Transportation Program (MATP) services and employment (referrals must be made from the Department of Public Welfare). CCA also offers services for individuals at risk of homelessness or low-income homeowners and provides rental assistance, utility assistance, weatherization services, and other forms of housing assistance. BeST Transit

BeST TRANSIT operates a Door-to-Door transportation program for Bradford, Sullivan and Tioga counties. There are general public fixed routes and door-to-door shared routes. “Shared Ride” means that the service is open to the public but is not a taxi service. The Shared Ride service will pick you up at your door and transport you to your destination. Since the service is “Shared Ride”, there may be other persons on the vehicle besides you or the driver may need to pick up other passengers prior to dropping you off at your destination. Door-to-door shared rides are open to seniors, people with disabilities, and people with a Medical Assistance (MA) Card. General public has to pay a fee each way. Bucks County Transport

Participants are required to apply for the service before scheduling a ride by completing either the senior or the general public application for the shared ride program.

Bucks County Transport is a private, non-profit organization that provides shared ride transportation services for all Bucks County residents. Seniors (ages 65+) receive a 15% discount on fares. All fares are available on their website for the shared-ride program. Before a trip can be scheduled, you have to register with Bucks County Transport either by calling 1-888-795-0740 or printing, completing, and mailing the application (which is available online). Scheduling hours are available 8:30AM-4:30PM and transportation is available Monday-Friday 6AM-6PM. Bucks County Transport asks for rides to be scheduled at least 2 weeks in advance.

Coalition to Shelter and Support the Homeless

Coalition to Shelter and Support the Homeless (CSSH) is an alliance of faith-based and community service organizations assisting unsheltered adults in Central Bucks County. Our first priority is to provide shelter on Code Blue nights. At the same time, we try to link the homeless and financially distressed to needed resources. CSSH only provides transportation to and from the shelter in the winter and to free community meals. There are free community meals the first, second, and third Sunday of the month. The Carbon-Monroe-Pike Mental Health and Developmental Services provides a wide range of services for individuals with mental health conditions and/or developmental disabilities. The Centre County Office of Aging provides services for seniors at six locations throughout the county. Medical Assistance Transportation Program

The Medical Assistance Transportation Program (MATP) provides transportation to and from medical appointments, and prescription and medical equipment pickups for individuals receiving Medical Assistance through the Pennsylvania Department of Human Services. MATP is available Monday through Friday from 7:00am-5:30pm. Ride appointments must be scheduled at least 24 hours in advance during office hours between 7:00am and 4:00pm, Monday through Friday. Individuals who receive Medical Assistance and live near public transit may be eligible for reimbursement of public transit fares through MATP. Individuals who receive Medical Assistance and use a personal vehicle to get to and from medical appointments may be eligible for gas mileage reimbursement. Interested individuals should contact Rover Community Transportation to register for the program.

Clarion Area Agency on Aging

Clarion Area Agency on Aging offers a wide range of programs and cooperates with many community agencies to provide a comprehensive network of services. The Farmer’s Market Nutrition Program provides eligible participants with $20 worth of vouchers to purchase fresh fruits and vegetables at farmer’s markets. Additional programs include individual case management; transportation; prime time health classes; community education; home cooked meals; legal assistance; and in-home supportive care and services. The Agency has four Senior Centers located throughout Clarion County. The centers offer lunch, and a variety of recreational, social and educational opportunities. Participants enjoy programs, trips and entertainment along with varied evening activities. Erie Metropolitan Transit Authority (EMTA)

Participants are required to obtain a Senior ID card from the EMTA and show proof of age in order to participate in the free transportation service.

By showing a Senior Identification card, anyone in Erie County can ride any EMTA bus for free. An application can be mailed or filled out at any of the EMTA officers. To receive the ID card, proof of age is required, which can be satisfied by the following documents: valid driver's license; birth certificate; armed forces discharge or separation papers; passport or naturalization papers; PACE card' statement of age verification by the Social Security Office; baptismal certificate; PennDot non-drivers license; veteran's universal access card; and resident alien card. Fayette Area Coordinated Transportation (FACT)

To be eligible for Medical Assistance Transportation, you must have a current medical assistance card; be a permanent or temporary resident of Fayette County; and need transportation to get a covered medical service from a provider who access the ACCESS card or is a health care provider in your network. Eligibility for coverage of shared ride trips is determined by the Fayette County Assistance Office (41 West Church Street Uniontown, PA 15401).

Fayette Area Coordinated Transportation (FACT) works to provide all Fayette County residents with quality public transportation services, regardless of age, income, or mobility requirements. Any Fayette County resident may use Shared Ride and all clients must pre-register for the service before requesting a trip. The fare depends on the purpose of the trip and if any of the various sources fund it. Clients may be eligible for one or more funding programs. Individuals aged 65 and older may use Shared Ride trips for a medical appointment, banking, grocery shopping, employment, post office, senior center, adult day care center, pharmacy, and visiting for immediate family members (spouse or children). Medical Assistance Transportation provides transportation for treatment, medical evaluation, or to purchase prescription drugs or medical equipment that is compensated by medical assistance.

Forest County Transportation Department

The Forest County Transportation Department provides free and low cost rides to Forest County residents. Individuals 65 and older receive free and discounted rides for medical, shopping, senior center, banking, etc. needs. Individuals on Medical Assistance with an Access Card and no registered vehicle within the household may ride for free to medical appointments only. Individuals with disabilities may ride for a small co-payment. The general public may also ride for a fee. Please check the website for more information about transportation fees. The Perry County Literacy Council (PCLC)

The mission of the Perry County Literacy Council (PCLC) is to provide educational programs for Perry County residents and their families that will enable them to better function as productive, responsible citizens.
