Adams County Office for Aging

The Adams County Office for Aging is a private non-profit agency dedicated to providing assistance to and advocacy for people age 60 and above within Adams County. Our mission is to assist people in maintaining independence and dignity within their own homes and communities. We offer a variety of services to support our age 60 and over population including; adult day care, senior centers, Lifeline systems, medical equipment loan, volunteer opportunities, tax preparation assistance, Medicare counseling, transportation, property tax and rent rebate assistance, protective services, legal services, and a variety of health and educational programs. For in-home care, individual can self-refer or a medical professional can refer as well. As for the other services, interested person should just call ahead and let them know they are coming.

Accelerated Support Services for Older Adults

Accelerated Support Services for Older Adults is located in the Hill House Senior Services Center, and provides case management and support services for formerly incarcerated individuals that 60 years and older. APPRISE Medicare Enrollment Consultation

APPRISE, Pennsylvania's State Health Insurance Program (SHIP), provides free expert Medicare counseling to individuals 65 and older. APPRISE was created by the Pennsylvania Department of Aging to advise Pennsylvania residents about the available health insurance benefits so that they can make informed decisions about their healthcare options. APPRISE provides personalized counseling (over the phone, via email, or in person) to participants in order identify their healthcare needs and preferences, and obtain Medicare and supplemental coverage that suits their lifestyle and budget. APPRISE is housed at Wesley Family Services.

Hill House Association

The Hill House offers a wide range of programs and services to help meet the needs of the community. Accelerated Support Services for Older Adults - A program providing supportive services to persons 60 years and older that have been released from jail or prison and need assistance obtaining housing, food, and clothing. First Source Center - Provides comprehensive workforce development services to citizens that lead to meaningful employment and positively impact the economic health of the community. Home-Delivered Meals - A nutrition program that provides daily meals to homebound older adults who are unable to prepare meals for themselves due to physical or medical reasons. EARN - To assist individuals presently receiving assistance from the Department of Public Welfare to secure and maintain gainful employment. Senior Community Services Center - The senior center provides a place for for seniors to come together within their community and socialize with friends and family. The center also provides resources that address vital concerns for seniors wishing to live independently.

North Hills Community Outreach

North Hills Community Outreach provides services to low-income individuals and families in Western Pennsylvania under four core values: recognizing the love of God for everyone, treating all individuals with dignity, fostering self-sufficiency, and managing resources in a responsible manner. Options/Care: Management Services for Independent Living

Options/Care Management, a program offered by the Family Services of Western Pennsylvania, is for individuals 60 years and older who need assistance remaining independent and living in their own homes. Care managers provide services based on a personalized assessment of the participant's psycho-social, environmental, and financial needs. Participants are eligible to receive home-delivered meals, resource counseling and support, monthly visits from a care manager, and more. Participants' families may also receive training and support through the Family Caregiver Support Program.

The Beaver County Office on Aging is committed to identifying the diverse needs of our aging population, providing quality services, preserving the dignity of the individual and advocating for the older person's rights. BeST Transit

BeST TRANSIT operates a Door-to-Door transportation program for Bradford, Sullivan and Tioga counties. There are general public fixed routes and door-to-door shared routes. “Shared Ride” means that the service is open to the public but is not a taxi service. The Shared Ride service will pick you up at your door and transport you to your destination. Since the service is “Shared Ride”, there may be other persons on the vehicle besides you or the driver may need to pick up other passengers prior to dropping you off at your destination. Door-to-door shared rides are open to seniors, people with disabilities, and people with a Medical Assistance (MA) Card. General public has to pay a fee each way. Family Service Association of Bucks County

Participants are required to be low-income older adults living in independent housing in Bucks County.

The Family Services Association of Bucks County offers Benefits Navigation for Older Adults, free face-to-face assistance with applying for public benefits to improve financial stability. The program helps low-income older adults apply for: SNAP (food stamps); LIHEAP (heating assistance); PACE/PACENet (help lower prescription costs); Extra Help/LIS (also helps lower prescription costs); Medical Savings Benefit (state pay your Medicare Part B premium); Medical Assistance (provide comprehensive, low-cost health care); Property Tax/Rent Rebate (receive a refund of taxes or rent paid in the prior year). Services are free of charge to low-income older adults living in independent housing in Bucks County. To access services, call the Family Services Association of Bucks County to do a preliminary evaluation of benefits eligibility by phone and then can schedule an appointment to meet at the individual's home or nearby location.

Clarion Area Agency on Aging

Clarion Area Agency on Aging offers a wide range of programs and cooperates with many community agencies to provide a comprehensive network of services. The Farmer’s Market Nutrition Program provides eligible participants with $20 worth of vouchers to purchase fresh fruits and vegetables at farmer’s markets. Additional programs include individual case management; transportation; prime time health classes; community education; home cooked meals; legal assistance; and in-home supportive care and services. The Agency has four Senior Centers located throughout Clarion County. The centers offer lunch, and a variety of recreational, social and educational opportunities. This resource provides private duty home care and home support services. All services are overseen by registered nurses. They can offer home support services and help with personal care, such as bathing, dressing and meal preparation, medication management, pet care, running errands, light housekeeping, and accompaniment services. Erie Metropolitan Transit Authority (EMTA)

Participants are required to obtain a Senior ID card from the EMTA and show proof of age in order to participate in the free transportation service.

By showing a Senior Identification card, anyone in Erie County can ride any EMTA bus for free. An application can be mailed or filled out at any of the EMTA officers. To receive the ID card, proof of age is required, which can be satisfied by the following documents: valid driver's license; birth certificate; armed forces discharge or separation papers; passport or naturalization papers; PACE card' statement of age verification by the Social Security Office; baptismal certificate; PennDot non-drivers license; veteran's universal access card; and resident alien card. The application can also be found on EMTA's website.

Participants are required to be over the age of 60.

The John F. Kennedy Senior Center provides community-based support for older individuals to live independently in their own home. Anyone 60 and over and their spouse (regardless of age) is eligible. Activities at the Senior Center include: cards, computers, walking club, volunteer opportunities, holiday parties, bingo, board games, fitness classes, health screens, day trips, bible study, and guest speakers. Fayette Area Coordinated Transportation (FACT)

To be eligible for Medical Assistance Transportation, you must have a current medical assistance card; be a permanent or temporary resident of Fayette County; and need transportation to get a covered medical service from a provider who access the ACCESS card or is a health care provider in your network. Eligibility for coverage of shared ride trips is determined by the Fayette County Assistance Office (41 West Church Street Uniontown, PA 15401).

Fayette Area Coordinated Transportation (FACT) works to provide all Fayette County residents with quality public transportation services, regardless of age, income, or mobility requirements. Any Fayette County resident may use Shared Ride and all clients must pre-register for the service before requesting a trip. The fare depends on the purpose of the trip and if any of the various sources fund it. Clients may be eligible for one or more funding programs. Individuals aged 65 and older may use Shared Ride trips for a medical appointment, banking, grocery shopping, employment, post office, senior center, adult day care center, pharmacy, and visiting for immediate family members (spouse or children). Medical Assistance Transportation provides transportation for treatment, medical evaluation, or to purchase prescription drugs or medical equipment that is compensated by medical assistance.

The Fulton County Medical Center (FCMC) is a Pennsylvania non-profit corporation which owns and operates a 21-bed critical access hospital and 67-bed nursing home facility. The Southwestern Pennsylvania Area Agency on Aging aims to promote the well-being of older adults in Greene, Washington, and Fayette Counties through a wide range of programs and services. Participants are required to be over the age of 60.

The Westmoreland County Area Agency on Aging provides services to older adults in Westmoreland County. The Westmoreland County AAA connects older adults with adult day care services, care management services, home health services, home-delivered meals, personal care services, health insurance, free legal services, senior centers (which provide meals, computer classes, social activities, and more), and transportation. Services may be provided at no cost, a sliding fee scale, or subject to some form of cost-sharing.

Pre-Application for PHA Senior Citizen Housing

Participants are required to be 55 or older; some options for citizens aged 55 and older and their families.

The Pre-Application for PHA Senior Citizen Housing is a form that must be completed and returned to the Philadelphia Housing Authority in order to start the process of securing public housing assistance for senior citizens. The application can be submitted at the Admissions office at 712 N. 16th St (16th and Fairmount). Applicants must also select which site interests them. Senior citizens with special needs may request assistive equipment under the Reasonable Accommodations section.
