The Adams County Office for Aging is a private non-profit agency dedicated to providing assistance to and advocacy for people age 60 and above within Adams County. Our mission is to assist people in maintaining independence and dignity within their own homes and communities. We offer a variety of services to support our age 60 and over population including; adult day care, senior centers, Lifeline systems, medical equipment loan, volunteer opportunities, tax preparation assistance, Medicare counseling, transportation, property tax and rent rebate assistance, protective services, legal services, and a variety of health and educational programs. For in-home care, individual can self-refer or a medical professional can refer as well. As for the other services, interested person should just call ahead and let them know they are coming.

The Hill House offers a wide range of programs and services to help meet the needs of the community. Accelerated Support Services for Older Adults - A program providing supportive services to persons 60 years and older that have been released from jail or prison and need assistance obtaining housing, food, and clothing. First Source Center - Provides comprehensive workforce development services to citizens that lead to meaningful employment and positively impact the economic health of the community. Home-Delivered Meals - A nutrition program that provides daily meals to homebound older adults who are unable to prepare meals for themselves due to physical or medical reasons. EARN - To assist individuals presently receiving assistance from the Department of Public Welfare to secure and maintain gainful employment. Senior Community Services Center - The senior center provides a place for for seniors to come together within their community and socialize with friends and family. The center also provides resources that address vital concerns for seniors wishing to live independently.

Options/Care Management, a program offered by the Family Services of Western Pennsylvania, is for individuals 60 years and older who need assistance remaining independent and living in their own homes. Care managers provide services based on a personalized assessment of the participant's psycho-social, environmental, and financial needs. Participants are eligible to receive home-delivered meals, resource counseling and support, monthly visits from a care manager, and more. Participants' families may also receive training and support through the Family Caregiver Support Program.

The Armstrong County Area Agency on Aging offers programs and services for older adults residing in Armstrong County. The AAA connects individuals to seniors centers, housing, home delivered meals, legal assistance, in-home services, adult day care services, medical insurance counseling, transportation services, employment training and placement services, and more.

The Beaver County Office on Aging is committed to identifying the diverse needs of our aging population, providing quality services, preserving the dignity of the individual and advocating for the older person's rights. Programs and services available are the Apprise Program, health and insurance coverage counseling; senior garden plots; in-home services; care management services; adult day cares; senior centers; transportation assistance; home delivered meals; healthy living classes; etc. There are several senior centers throughout Beaver County that offer fitness classes, field trips, recreational games, fellowship, educational activities and hot, nutritious meals for those 60 years and older. The Huntingdon-Bedford-Fulton Area Agency on Aging provides services for older adults residing in Huntingdon, Bedford, and Fulton counties. The AAA provides assistance with accessing health insurance, rent and property tax rebates, meals, transportation (Shared Ride Program transportation and reduced fares), in-home services, home support, senior centers, and more. Please note that the AAA has offices in all three counties.

Hyndman Area Health Center provides medical, dental, and laboratory services. Hyndman Area Health Center also offers sliding scale discounts for medical services, discounts on prescription medications, and assists individuals with accessing health care through the marketplace. Hyndman Area Health Center accepts Medicaid, Medicare, and commercial health insurance plans. Please look online or call ahead to find out which office location is most convenient.

The Berks Community Health Center (BCHC) provides physical care services, mental health services, dental services, and other specialized health services to patients of all ages. BCHC also assists individuals with accessing health insurance through the marketplace. There is a sliding fee program to assist individuals with covering health care costs and BCHC has full-time Spanish translators on staff. BCHC offers services at three medical centers in Berks County.

The Berks County Area Agency on Aging (AAA) provides services to older adults in Berks County. The Berks County AAA connects older adults with home delivered meals, adult daily living centers, senior community centers, transportation services, public health insurance, assistance for older adults who are raising minors, in-home supportive services (including home health services), health and wellness programs, and more.

Blair Senior Services is a non-profit corporation that aims to meet the needs of older adults in Blair County. This includes: case management, transportation, daily meals, educational and recreational activities at four different senior centers, abuse and abandonment protective services, and legal advocacy (Power of Attorney and guardianship services, wills and, long-term care complaints).

The B/S/S/T Area Agency on Aging, Inc. is a non-profit agency that serves as a resource for all seniors and their families in Bradford, Sullivan, Susquehanna, and Tioga Counties in Pennsylvania. Our goal is to provide information and connections to services to help people remain an independent as possible, for as long as possible.

The Ann Silverman Community Health Clinic provides free medical and dental care for uninsured low-income adults and children residing in Bucks County. The clinic also providers information about other community resources and helps uninsured apply for healthcare coverage.

The Butler County Area Agency on Aging provides services to Butler County residents. Services include home-delivered meals, counseling, health insurance counseling, legal assistance, meals, activities, and educational programs through senior centers, health programs, and transportation through Butler Area Rural Transit (BART).

The Community Health Clinic of Butler County provides primary health, dental care, and diabetic and nutritional counseling to individuals residing in Butler County (between the ages of 19-64 years old) who do not have insurance and meet certain income requirements. The mission of Highlands Health is to provide free medical and pharmaceutical care to the low income uninsured population in the Laurel Highlands region of Western PA. A person may receive free doctor check-ups, free medicine, free X-rays, free blood tests, and a variety of other basic tests. Naloxone, also called Narcan and Evzio, an easy-to-use prescription medicine to reverse opioid overdose, is available free of charge, no questions asked. To be a patient at the Free Clinic a person cannot have any health insurance. He/she cannot have medical assistance from the Pennsylvania Department of Welfare or be eligible for Medicare. The person must have a photo ID and show proof of total household income. The person will be required to obtain a denial letter from the Medical Assistance Office within 90 days. The person will have an Intake appointment to determine if he/she is eligible for clinic services before seeing the doctor. Additional forms to fill out are available on the website. All appointments must be scheduled in advance.

The Office of Human Services (OHS) provides a wide range of services to older adults in Cameron, Elk, and McKean Counties. OHS assists older adults with accessing public health insurance counseling, in-home personal care services, nursing home transition services, counseling, transportation, in-home nursing services, senior centers, and more.

Carbon Medical Associates provides medical services (general internal medicine and family medicine, with physicians who have expertise with diabetes and hypertension). Carbon Medical Associates accepts a variety of private health insurance plans and Medicare, but does not accept Medicaid. Please note that there are two office locations in Carbon County.

Centre Volunteers in Medicine (CVIM) serves and advocates for the medically underserved living or working in Centre County. CVIM provides medical and dental care, case management and necessary medications at no cost to qualified uninsured and under-insured residents of Centre County and those working in Centre County who live in neighboring counties that do not have free clinics or programs to meet their medical needs. To qualify for care with us: *You must be living or working in Centre County for at least 7 days. *You must NOT have insurance for the services for which you are applying (This includes Adult Basic, Access Card/Medicaid, Medicare, Veterans Benefits). *You must meet income guidelines. Residents must have a household income at or below 250% of the Federal Poverty Level.

Community Volunteers in Medicine (CVIM) offers free coordinated health care. CVIM offers physical and dental services, as well as referrals to a network of local physicians, hospitals, and other healthcare providers to offer comprehensive care.

La Comunidad Hispana offers individuals supports such as physical exams, wellness visits, behavioral health counseling, testing and treatment for treatment of sexually transmitted diseases, dental care, and referrals to specialty care. La Comunidad Hispana also offers dental services, free ESL classes, GED classes, citizenship classes, and job placement services. Additionally, La Comunidad offers support to apply for public benefits and legal consultation.

The Primary Health Network Clarion Community Health Center, located in Clarion, PA, opened in October 2012, offering patients access to Family Medicine services. As one of PHN's multi-service community health centers, this site also provides Behavioral Health, Dental, and Urgent Care services.

The Clearfield County Area Agency on Aging provides services for older adults in Clearfield County. The Clearfield County Area Agency on Aging connects older adults with senior centers, health insurance, home-delivered meals, educational programs through the Lifelong Learning Institute, information and referrals to external services, in-home care services, caregiver support, and more.

The Free Medical Clinic of DuBois provides medical care and pharmaceuticals to uninsured individuals in the DuBois area. The essence of our mission to the uninsured is to provide medical care with the same dignity and confidentiality afforded to the private sector in an easily accessible facility.

The Lycoming-Clinton Counties for Community Action (STEP) Office of Aging provides assistance to older adults in Lycoming and Clinton Counties. STEP assists older adults with accessing home-delivered meals, senior centers, transportation services, health screenings and education, in-home services, health insurance counseling, legal services, case management services, assistance with transferring to or from a personal care facility, and more. Please note that there is a STEP office in both Lycoming and Clinton counties.

The Columbia-Montour Aging Office serves older adults in Columbia and Montour Counties. The Columbia-Montour Aging Office provides care-managed services, home health services, nursing home transition services, in-home services, adult day care services, access to senior centers, meals, health insurance counseling, legal assistance, free transportation services, and more. Please note that an individual may have to travel to Columbia County to receive certain services.

Active Aging, Inc. offers numerous services to individuals age 60 and above. They have six senior centers which provide meals, transportation, educational programs and wellness activities. They also have in-home services which provide personalized assessments, case management, home delivered meals and more. There are also counselors who provides public health insurance counseling.

Meadville Medical Center is a private, community hospital with many locations throughout Meadville. They offer a full range of health services and specialists and they accept all insurances, including Medicare and Medicaid.

Cumberland Aging and Community Services helps to connect older adults in Cumberland County with transportation services, senior centers, home-delivered meals, adult day service centers, health insurance counseling, caregiver support, and more.

Sadler Health Center specializes in providing services to the uninsured, underinsured, Medicare and Medicaid eligible individuals. Sadler provides Medical, Dental, Behavioral Health and Tobacco Cessation as well as Prescriptive Services and the Nurse Family Partnership program. Sadler Health Center is staffed with a team of qualified professionals including Family Physicians, Nurse Practitioners, Licensed Clinical Social Workers, Dentists and Dental Hygienists. These professionals are committed to providing the highest quality of care to their patients.

Alder Health Services offers LGBT-Friendly health services in the Harrisburg area. Alder Health Services provides primary care services, behavioral health supports for individuals who are struggling with anxiety, depression, gender identity, affect and infected by HIV/AIDS, general or major life adjustments, trauma, sexuality, etc. Alder Health Services also offers STD/HIV testing and case management and support.

CRESC is a network of caring professionals and community leaders whose purpose is to provide encouragement, hope and resources for the formerly incarcerated individuals. CRESC provides a variety of services, including mentoring, housing, employment, and referrals to other resources.

The Dauphin County Area Agency on Aging provides various services for older adults. The Dauphin County AAA helps connect older adults with adult care programs, free civil legal services, transportation services, health classes (including healthy cooking, disease prevention, etc.), meal delivery services, senior centers, and health insurance counseling.

Hope Within Ministries offers a full array of primary health care services including complete physical exams and wellness/preventative care, acute and chronic disease management, assistance to patients in completing and submitting application for other programs where they meet eligibility criteria such as Medical Assistance, and referrals to other specialists. Hope Within Ministries also offers low-cost mental health services based on an income-based sliding scale. Please note that individuals in Dauphin County must travel to Elizabethtown to receive treatment.

The Partnership for Hope connects individuals with service at homeless shelters, housing facilities for low income and mentally challenged adults, and other faith-based ministries and community programs. They provide job training, recovery services, computer skills, and other supports for individuals returning from incarceration. There are also opportunities for volunteering.

ChesPenn Health Centers provide medical services, including preventive health care and social services to families and individuals on a sliding scale.

This resource provides private duty home care and home support services. All services are overseen by registered nurses. They can offer home support services and help with personal care, such as bathing, dressing and meal preparation, medication management, pet care, running errands, light housekeeping, and accompaniment services. Most insurance carriers including Medicare, Medicaid and private insurance plans will cover home healthcare that is deemed medically necessary.

The Office of Human Services (OHS) provides a wide range of services to older adults in Cameron, Elk, and McKean Counties. OHS assists older adults with accessing public health insurance counseling, in-home personal care services, nursing home transition services, counseling, transportation, in-home nursing services, senior centers, and more.

St. Paul's Neighborhood Free Clinic provides primary health and dental care. St. Paul’s Neighborhood Free Clinic is not a walk-in or emergency clinic, patients are seen by appointment only, and dental appointments are scheduled upon referral from the medical doctor. Contact the clinic to determine your eligibility for clinic services. It is important to have your financial information (past year plus current year) when you call.

Adagio Health offers a wide range of services including gynecology; family planning; breast and cervical cancer screening; and STI prevention, testing, and treatment. The health clinic places a special focus on caring for teens, the LGTBQ community, low income individuals, and those without insurance in a judgement free environment. Adagio also offers healthcare services for men, providing reproductive health exams, STI testing, and screenings for certain cancers. For women 40 years and older, breast and cervical cancer screenings are free. Cost is determined on a sliding scale fee by staff at the health clinic. All teens under 18 receive services for free.

Experience Inc. (Area Agency on Aging of Warren and Forest Counties) assists older adults with accessing a wide range of programs and services. Experience Inc. facilitates access to meal delivery services, personal care and home support, housing assistance, case management services, senior centers, transportation, educational classes, counseling and support groups, social activities, and more. Services are offered at multiple locations.

The Franklin County Area Agency on Aging connects older adults in Franklin County with health insurance, home-delivered meals, senior centers, in-home care services, adult day care services, mental health services, legal services, personal care, nursing home transition assistance, and more.

Summit Health offers physical and behavioral health services in hospitals, health clinics, and specialty offices in Franklin County. Services include inpatient and outpatient mental health and substance use services, primary care services, rehabilitation services, women's health services, and more. The Fulton County Medical Center (FCMC) is a Pennsylvania non-profit corporation which owns and operates a 21-bed critical access hospital and 67-bed nursing home facility. FCMC provides a range of medical services from cardiology to respiratory to rehabilitative to long term care and many other specialized health care services. FCMC runs a Community Health and Wellness Center that provides free health screenings and fitness classes. The Center is located at 374 Lincoln Way West, McConnellsburg, PA 17233.

The Huntingdon-Bedford-Fulton Area Agency on Aging provides services for older adults residing in Huntingdon, Bedford, and Fulton counties. The Southwestern Pennsylvania Area Agency on Aging aims to promote the well-being of older adults in Greene, Washington, and Fayette Counties through a wide range of programs and services. The agency assists older adults with accessing transportation, senior centers, care management, health and wellness programs, adult day care, free legal services (including assistance with Supplemental Security Income (SSI), Social Security benefits, public health insurance), and more. Huntingdon-Bedford-Fulton Area Agency on Aging is a public agency under the jurisdiction of the Counties of Huntingdon, Bedford & Fulton. The agency is dedicated to serving the elderly of Huntingdon, Bedford, and Fulton Counties and provides a variety of in-home services; specialized services; transportation services; PACE and PACENET Programs; Property Tax and Rent Rebate; and Low-Income Home Energy Assistance Program. Aging Services Inc. provides a variety of services and programs to older adults in Indiana County. Aging Services assists older adults with accessing transportation, meals, home support, life skills education at adult day care centers, in-home medical services, placement services, public health insurance counseling, and more. Aging Services Inc. uses a cost-sharing system and costs vary based on the desired services and an individual's income. (Therefore, certain services are free for eligible individuals.)

Primary Health Networks - Indiana Dental Center offers dental services. Primary Health Networks accepts all patients regardless of their insurance status or ability to pay, and offers a sliding fee scale program. To qualify for a sliding fee scale, individuals must provide proof of income for all individuals in their household.

The Mifflin-Juniata Area Agency on Aging provides a variety of services to older adults in Juniata and Mifflin Counties. The Mifflin-Juniata Area Agency on Aging connects older adults with home-delivered meals, in-home care, nursing home transition services, senior centers, care management, transportation services, health insurance counseling, and more. Please note that the main office is located in Mifflin County.

The Lackawanna County Area Agency on Aging provides services to older adults in Lackawanna County. The Lackawanna County Area Agency on Aging assists older adults with accessing senior community centers (which offer computer classes and social activities), adult daily living centers, in-home care services, health insurance, home-delivered meals, transportation services, and more.

The Wright Center is a non-profit, community-based graduate medical education consortium and health care provider. The Wright Center provides a range of primary care, oral health care, behavioral health, counseling, and infectious disease services (including services specifically for individuals with HIV) . Please note that the Wright Center provides services from multiple locations in Lackawanna County. Patients are welcome regardless of health insurance status or ability to pay for services.

United Neighborhood Centers of Northeastern Pennsylvania (UNC) provides a variety of services and programs to low-income community members. UNC provides one-on-one health care coordination, older adult community centers, child care centers, housing assistance and emergency utility services, adult education, one-on-one tutoring, computer labs, food and clothing pantries, and opportunities to volunteer. Since 2001 Beginning In the Right Direction, B.I.R.D. Ministries has been a lifeline of support and encouragement to women trapped in the cycle of prison, addiction and abuse. They provide all that is needed while helping women who have been in prison rebuild their lives. Through a network of resources- from social services agencies to faith-based ministries and churches- they are able to provide the women and their families with the support and services they so desperately need.

Dental Access of Lancaster County (DALCO) is a program that provides dental health services for low-income, uninsured patients. DALCO provides ongoing care, including routine cleanings, to keep your teeth healthy.

Hope Within Ministries offers a full array of primary health care services including complete physical exams and wellness/preventative care, acute and chronic disease management, assistance to patients in completing and submitting application for other programs where they meet eligibility criteria such as Medical Assistance, and referrals to other specialists. GEO Reentry provides flexible options such as residential housing and treatment, day reporting programs, and inpatient treatment and training for individuals returning from incarceration.

The Lancaster County Office of Aging provides services to older adults in Lancaster County. The Office of Aging connects older adults with transportation, home-delivered meals, health insurance counseling, senior centers, in-home care services, placement services, counseling, and more.

Lancaster Health Center provides healthcare services to all, regardless of one’s ability to pay. Their Sliding Fee Program offers a discounted rate on medical and dental services offered at their facilities. In addition to the Sliding Fee Program, facilities provide convenient parking and easy access for patients with a physical disability, as well as materials in both English and Spanish, and interpretive services. WellSpan Philhaven offers services for all ages and various levels of care, including hospitalization, residential and day programs, outpatient counseling, and services in the home, school and community. They have multiple locations, so please look online or call to find out which office is nearest to you.

The RASE Project provides substance use treatment services and supports the recovery process. The RASE Project offers free training and educational workshops that address addiction, the family, and the recovery process. They also operate transitional living facilities for women to support their recovery.

The Spanish American Civic Association (SACA) offers extensive drug and alcohol services (both inpatient and outpatient), counseling services, HIV/AIDS services. At the Tec Centro, individuals are provided with services to remove any barriers that may inhibit career advancement by offering adults education completion, job readiness workshops, employment services, case management, bilingual training, and multiple career pathways. Among these career pathways are: CNA medical receptionist, medical billing, dental assisting, business and hospitality, construction, manufacturing, and general office (clerical). SACA also has a senior center which offers meals, various activities, and case management services (including housing placement and medical management) to individuals over 60. Please note that services are offered in multiple locations.

Water Street Mission provides services to low income individuals and people experiencing homelessness. All guests have access to free medical and dental services, as well as behavioral health, spiritual and career support. To receive services, individuals must meet certain qualifications to ensure that the quality care is going to those in most need.

Challenges Center is an innovative community facility offering a wide variety of educational, recreational and health-related activities to help support the independence of adults ages 50+ who live in Lawrence County. While many of the Center's offerings are free, others require membership and/or separate fees.

Lawrence County Community Action Partnership is made up of three agencies that seek to involve the community in assessing local needs and attacking the causes and conditions of poverty. They are: Lawrence County Social Services, Inc., United Community Services of Lawrence County, Inc., and Allied Coordinated Transportation Services, Inc. Lawrence County Community Action Partnership reaches out to low-income people to address their multiple needs through a comprehensive approach, develops partnerships with other community organizations, involves clients in agency's operations, and administers a full range of coordinated programs designed to have a measurable impact on poverty. Among its many services, Lawrence County Community Action Partnership provides support in the following areas: Education; employment; emergency and community services; individual and family services; housing rehabilitation; senior services; transportation; and youth programs.

The mission of the Lebanon County Area Agency on Aging is to develop, coordinate and promote a continuum of services that values and supports older adults as they age; empowers them to maintain independence, dignity and respect; and enhances their overall well-being and quality of life.

Lehigh County Office Of Aging and Adult Services provides a variety of services to older adults in Lehigh County. The Office of Aging and Adult Services connects individuals to home delivered meals, assessment and care management, nursing home transition, case management services, senior centers (which provide meals, social activities, computer classes, and more), housing services, adult day centers, public health insurance counseling, personal care services, transportation services, and more.

New Bethany Ministries provides food support, hygiene support, and wellness programing at their Mollard Hospitality Center. The Hospitality Center serves a continental or hot breakfast each weekday morning; and the food pantry provides food daily, and clothing, hygiene items, and household items when they are available. They also offer free, public showers and access laundry machines. Twice a month they provide holistic services through a free primary care clinic.

Victory House aims to provide shelter services and supportive services to men and veterans who are homeless or reentering society. Victory House offers three meals daily, emergency shelter and transitional housing, case management and counseling, addiction prevention assistance, access to a health center, employment assistance, housing placement assistance, life skills education, community giveback participation, and aftercare.

The Freeland Health Center provides medical care for children and adults. Most commercial insurances, Medical Assistance, and Medicare are accepted; uninsured or underinsured patients are eligible for to pay for services on a sliding scale. The Lycoming-Clinton Counties for Community Action (STEP) Office of Aging provides assistance to older adults in Lycoming and Clinton Counties. STEP assists older adults with accessing home-delivered meals, senior centers, transportation services, health screenings and education, in-home services, health insurance counseling, legal services, case management services, assistance with transferring to or from a personal care facility, and more. The West End Christian Community Center is the outreach ministry of Saint John's-Newberry UMC. The West End Community Center offers community meals, walker, canes, crutches, and wheelchair loans (depending on availability), an optical clinic (income requirements apply), and an Alcoholics Anonymous support group. There is also a food pantry for individuals who live in certain areas of Lycoming County (individuals are required to provide proof of income, birth dates, Social Security Numbers, and photo identification). The Monroe County Area Agency on Aging assists older adults in Monroe County with accessing senior centers, home-delivered meals, protective services, personal care services, care management, health insurance counseling, and transportation.

The Montgomery County Office of Aging and Adult Services (AAS) provides services to older adults in Montgomery County. Most residents come to Project HOME from a safe haven, a shelter or from the streets. The PA Health Law Project works to ensure everyone has access to health care. Outside of advocating for the right to health care, the Law Project provides free legal services to any low-income resident of Pennsylvania having difficulty accessing publicly funded health care, whose health care has been terminated or reduced, or who have been denied Medicare (or similar health care programming). Additionally, the Law Project helps eligible residents apply for Medicare, responds to issues of denial of service (i.e. for prescriptions), and anyone struggling with their health care costs.
