The Adams County Office for Aging is a private non-profit agency dedicated to providing assistance to and advocacy for people age 60 and above within Adams County. Our mission is to assist people in maintaining independence and dignity within their own homes and communities. We offer a variety of services to support our age 60 and over population including; adult day care, senior centers, Lifeline systems, medical equipment loan, volunteer opportunities, tax preparation assistance, Medicare counseling, transportation, property tax and rent rebate assistance, protective services, legal services, and a variety of health and educational programs. For in-home care, individual can self-refer or a medical professional can refer as well. As for the other services, interested person should just call ahead and let them know they are coming.

APPRISE, Pennsylvania's State Health Insurance Program (SHIP), provides free expert Medicare counseling to individuals 65 and older. APPRISE was created by the Pennsylvania Department of Aging to advise Pennsylvania residents about the available health insurance benefits so that they can make informed decisions about their healthcare options. APPRISE provides personalized counseling (over the phone, via email, or in person) to participants in order identify their healthcare needs and preferences, and obtain Medicare and supplemental coverage that suits their lifestyle and budget. APPRISE is housed at Wesley Family Services.

Community Care helps individuals find service providers with the goal of improving the health and well-being of the community through the delivery of effective, cost-efficient, and accessible behavioral health services.

The Foundation of HOPE is an interfaith non-profit with the mission to “Empower incarcerated and released individuals to restore their relationship with their God, rebuild their lives, and reconcile to their communities.” The HOPE Aftercare Program is a pre- and post-release adult reintegration program for inmates and reentrants that seeks to ensure their successful return to the community. HOPE Aftercare provides resources, referrals, and guidance regarding employment, housing, education, food assistance, mental health services, and clothing. The program’s mentoring component matches inmates and reentrants with mentors who can assist them on a one-on-one basis.

The Hill House offers a wide range of programs and services to help meet the needs of the community. Accelerated Support Services for Older Adults - A program providing supportive services to persons 60 years and older that have been released from jail or prison and need assistance obtaining housing, food, and clothing. First Source Center - Provides comprehensive workforce development services to citizens that lead to meaningful employment and positively impact the economic health of the community. Home-Delivered Meals - A nutrition program that provides daily meals to homebound older adults who are unable to prepare meals for themselves due to physical or medical reasons. EARN - To assist individuals presently receiving assistance from the Department of Public Welfare to secure and maintain gainful employment. Senior Community Services Center - The senior center provides a place for for seniors to come together within their community and socialize with friends and family. The center also provides resources that address vital concerns for seniors wishing to live independently.

Options/Care Management, a program offered by the Family Services of Western Pennsylvania, is for individuals 60 years and older who need assistance remaining independent and living in their own homes. Care managers provide services based on a personalized assessment of the participant's psycho-social, environmental, and financial needs. Participants are eligible to receive home-delivered meals, resource counseling and support, monthly visits from a care manager, and more. Participants' families may also receive training and support through the Family Caregiver Support Program.

The Armstrong County Area Agency on Aging offers programs and services for older adults residing in Armstrong County. The AAA connects individuals to seniors centers, housing, home delivered meals, legal assistance, in-home services, adult day care services, medical insurance counseling, transportation services, employment training and placement services, and more.

Laurel Legal Services provides free civil legal services to low-income individuals and families. Laurel Legal Services provides housing and utility assistance, employment assistance, and assistance with accessing government benefits, and community education on legal issues. Laurel Legal Services has multiple offices.

Residents of Pennsylvania can seek assistance and a range of services for themselves and their families from professionally trained staff members at county assistance offices. You can also apply for and renew your benefits from the comfort of your home using COMPASS, the online resource for cash assistance, the Supplemental Nutrition Assistance Program (SNAP), help with child care, health care coverage, home heating assistance (LIHEAP), school meals, SelectPlan for Women and long-term living services.

The Beaver County Office on Aging is committed to identifying the diverse needs of our aging population, providing quality services, preserving the dignity of the individual and advocating for the older person's rights. Programs and services available are the Apprise Program, health and insurance coverage counseling; senior garden plots; in-home services; care management services; adult day cares; senior centers; transportation assistance; home delivered meals; healthy living classes; etc. There are several senior centers throughout Beaver County that offer fitness classes, field trips, recreational games, fellowship, educational activities and hot, nutritious meals for those 60 years and older. The Huntingdon-Bedford-Fulton Area Agency on Aging provides services for older adults residing in Huntingdon, Bedford, and Fulton counties. The AAA provides assistance with accessing health insurance, rent and property tax rebates, meals, transportation (Shared Ride Program transportation and reduced fares), in-home services, home support, senior centers, and more. Please note that the AAA has offices in all three counties.

Hyndman Area Health Center provides medical, dental, and laboratory services. Hyndman Area Health Center also offers sliding scale discounts for medical services, discounts on prescription medications, and assists individuals with accessing health care through the marketplace. Hyndman Area Health Center accepts Medicaid, Medicare, and commercial health insurance plans. Please look online or call ahead to find out which office location is most convenient.

The Berks Community Health Center (BCHC) provides physical care services, mental health services, dental services, and other specialized health services to patients of all ages. BCHC also assists individuals with accessing health insurance through the marketplace. There is a sliding fee program to assist individuals with covering health care costs and BCHC has full-time Spanish translators on staff. BCHC offers services at three medical centers in Berks County.

The Berks County Area Agency on Aging (AAA) provides services to older adults in Berks County. The Berks County AAA connects older adults with home delivered meals, adult daily living centers, senior community centers, transportation services, public health insurance, assistance for older adults who are raising minors, in-home supportive services (including home health services), health and wellness programs, and more.

The B/S/S/T Area Agency on Aging, Inc. is a non-profit agency that serves as a resource for all seniors and their families in Bradford, Sullivan, Susquehanna, and Tioga Counties in Pennsylvania. Our goal is to provide information and connections to services to help people remain an independent as possible, for as long as possible.

Bradford County Human Services is the name given to the combined county agencies of Child Care Information System, Children and Youth Services, Drug and Alcohol, Mental Health, and Intellectual Disabilities. This office also administers the Human Services Development Fund and Homeless Assistance Program for the County Commissioners. The county office offers financial assistance, food stamps, health care and medical assistance, help for heating and energy bills, employment and training counseling, information and referral for social services and government programs.

The Family Services Association of Bucks County offers Benefits Navigation for Older Adults, free face-to-face assistance with applying for public benefits to improve financial stability. The program helps low-income older adults apply for: SNAP (food stamps); LIHEAP (heating assistance); PACE/PACENet (help lower prescription costs); Extra Help/LIS (also helps lower prescription costs); Medical Savings Benefit (state pay your Medicare Part B premium); Medical Assistance (provide comprehensive, low-cost health care); Property Tax/Rent Rebate (receive a refund of taxes or rent paid in the prior year). Services are free of charge to low-income older adults living in independent housing in Bucks County. To access services, call the Family Services Association of Bucks County to do a preliminary evaluation of benefits eligibility by phone and then can schedule an appointment to meet at the individual's home or nearby location.

The Butler County Area Agency on Aging provides services to Butler County residents. Services include home-delivered meals, counseling, health insurance counseling, legal assistance, meals, activities, and educational programs through senior centers, health programs, and transportation through Butler Area Rural Transit (BART).

The mission of the Cambria County Area Agency on Aging is to enhance the quality of life of older citizens of Cambria County by promoting their well-being, dignity, independence and safety through compassionate sponsorship of social service programs for aging adults. The Area Agency on Aging provides the following programs: Adult Day Care, Aging and Disability Resource Center (ADRC), Assessments, Care Management, Congregate Meals, Domiciliary Care, Family Caregiver Support Program, Foster Grandparents Program, Guardianship, Health Insurance Counseling, Home Delivered Meals, Home Security Program, Home Support, Information & Assistance, Nursing Home Transition, OBRA, Ombudsman, Protective Services, Personal Assistance Services, Personal Care, Senior Activities Centers, Transportation, Waiver.

Northern Tier Community Action Corporation provides services to low-income individuals. Services include assistance with accessing public benefits (including SNAP, medical assistance, LIHEAP), housing assistance (including payment of rent and utilities), case management services, food assistance, and referrals.

The Office of Human Services (OHS) provides a wide range of services to older adults in Cameron, Elk, and McKean Counties. OHS assists older adults with accessing public health insurance counseling, in-home personal care services, nursing home transition services, counseling, transportation, in-home nursing services, senior centers, and more.

The Carbon County Area Agency on Aging helps individuals who are 60+ remain active in their communities by providing case management, transportation, volunteer opportunities, legal assistance, public benefit counseling, assistance with public benefit applications, access to senior centers and recreational activities, and in-home services.

The Centre County Office of Aging provides services for seniors at six locations throughout the county. The services include: health insurance counseling for Medicare recipients, home delivered meals, investigations of abuse, neglect, exploitation or abandonment of older adults, long-term care services, nursing home transition services, and casework services. The Office also provides funding to outside organizations for personal care services, transportation, adult day services, and legal assistance for those who qualify.

La Comunidad Hispana offers individuals supports such as physical exams, wellness visits, behavioral health counseling, testing and treatment for treatment of sexually transmitted diseases, dental care, and referrals to specialty care. La Comunidad Hispana also offers dental services, free ESL classes, GED classes, citizenship classes, and job placement services. Additionally, La Comunidad offers support to apply for public benefits and legal consultation.

Clarion Area Agency on Aging offers a wide range of programs and cooperates with many community agencies to provide a comprehensive network of services. The Farmer’s Market Nutrition Program provides eligible participants with $20 worth of vouchers to purchase fresh fruits and vegetables at farmer’s markets. Additional programs include individual case management; transportation; prime time health classes; community education; home cooked meals; legal assistance; and in-home supportive care and services. The Agency has four Senior Centers located throughout Clarion County. The centers offer lunch, and a variety of recreational, social and educational opportunities. Participants enjoy programs, trips and entertainment along with varied evening activities. The Clearfield County Area Agency on Aging provides services for older adults in Clearfield County. The Clearfield County Area Agency on Aging connects older adults with senior centers, health insurance, home-delivered meals, educational programs through the Lifelong Learning Institute, information and referrals to external services, in-home care services, caregiver support, and more.

The Lycoming-Clinton Counties for Community Action (STEP) Office of Aging provides assistance to older adults in Lycoming and Clinton Counties. STEP assists older adults with accessing home-delivered meals, senior centers, transportation services, health screenings and education, in-home services, health insurance counseling, legal services, case management services, assistance with transferring to or from a personal care facility, and more. Please note that there is a STEP office in both Lycoming and Clinton counties.

The Columbia-Montour Aging Office serves older adults in Columbia and Montour Counties. The Columbia-Montour Aging Office provides care-managed services, home health services, nursing home transition services, in-home services, adult day care services, access to senior centers, meals, health insurance counseling, legal assistance, free transportation services, and more. Please note that an individual may have to travel to Columbia County to receive certain services.

Active Aging, Inc. offers numerous services to individuals age 60 and above. They have six senior centers which provide meals, transportation, educational programs and wellness activities. They also have in-home services which provide personalized assessments, case management, home delivered meals and more. There are also counselors who provides public health insurance counseling.

Cumberland Aging and Community Services helps to connect older adults in Cumberland County with transportation services, senior centers, home-delivered meals, adult day service centers, health insurance counseling, caregiver support, and more.

Tri-County Community Action provides vocational, life skills, and counseling assistance to low-income individuals. A career training program provides vocational training in welding, manufacturing, and other trades, and completion provides a certificate in job readiness. Completion of vocational training will be supplemented by job search assistance with a case manager. Case managers and clients can work together for up to five years or until clients have achieved self-sufficiency. Case managers also lead group life skills classes and employment development workshops (including resume writing, interviewing, and job searches). The Housing & Credit Counseling program provides housing counselors and classes assisting in home buying, credit counseling, financial fitness, and lending basics. Walk-ins are accepted, but calling ahead is preferred. Tri County Community Action has Cumberland County offices in Shippensburg and Enola, and offices in Dauphin and Perry Counties.

CRESC is a network of caring professionals and community leaders whose purpose is to provide encouragement, hope and resources for the formerly incarcerated individuals. CRESC provides a variety of services, including mentoring, housing, employment, and referrals to other resources.

The Dauphin County Area Agency on Aging provides various services for older adults. The Dauphin County AAA helps connect older adults with adult care programs, free civil legal services, transportation services, health classes (including healthy cooking, disease prevention, etc.), meal delivery services, senior centers, and health insurance counseling.

Northern Tier Community Action Corporation provides services to low-income individuals. Services include assistance with accessing public benefits (including SNAP, medical assistance, LIHEAP), housing assistance (including payment of rent and utilities), case management services, food assistance, and referrals. Please note that individuals in Elk County will have to travel to Cameron County to receive housing assistance, but there are multiple food banks available in Elk County.

The Office of Human Services (OHS) provides a wide range of services to older adults in Cameron, Elk, and McKean Counties. OHS assists older adults with accessing public health insurance counseling, in-home personal care services, nursing home transition services, counseling, transportation, in-home nursing services, senior centers, and more.

The Erie County Assistance Office provides individuals and their families with a assistance and a range of services. Trained staff at the office can help individuals apply for and renew benefits for cash assistance, the Supplemental Nutrition Assistance Program (SNAP), help with health care coverage, home heating assistance (LIHEAP), school meals, SelectPlan for Women, and long-term living services.

Case managers at the Salvation Army through one-to-one interviews work to help clients with utility assistance (various energy bill grant programs) and food assistance (SNAP food programs).

North Western Legal Services (NWLS) provides free advice and legal representation to low-income individuals. NWLS provides assistance and advice related to utility terminations, vocational rehabilitation, appealing the denial of public health insurance, housing, accessing government benefits after receiving a denial or termination notice, accessing Social Security benefits after receiving a denial or termination notice, and more. The Franklin County Area Agency on Aging connects older adults in Franklin County with health insurance, home-delivered meals, senior centers, in-home care services, adult day care services, mental health services, legal services, personal care, nursing home transition assistance, and more.

The agency is dedicated to serving the elderly of Huntingdon, Bedford, and Fulton Counties and provides a variety of in-home services; specialized services; transportation services; PACE and PACENET Programs; Property Tax and Rent Rebate; and Low-Income Home Energy Assistance Program. The Salvation Army is an evangelical part of the universal Christian Church. The progam's message is based on the Bible and every program is rooted in service. Among its principle ministry services, the Huntingdon Worship & Service Center offers rental assistance, food boxes, volunteer opportunities, and case management services that can meet the individual needs of the client.

Aging Services Inc. provides a variety of services and programs to older adults in Indiana County. Aging Services assists older adults with accessing transportation, meals, home support, life skills education at adult day care centers, in-home medical services, placement services, public health insurance counseling, and more. Aging Services Inc. uses a cost-sharing system and costs vary based on the desired services and an individual's income. (Therefore, certain services are free for eligible individuals.)

The Lackawanna County Area Agency on Aging provides services to older adults in Lackawanna County. The Lackawanna County Area Agency on Aging assists older adults with accessing senior community centers (which offer computer classes and social activities), adult daily living centers, in-home care services, health insurance, home-delivered meals, transportation services, and more.

The Lancaster County Office of Aging provides services to older adults in Lancaster County. The Office of Aging connects older adults with transportation, home-delivered meals, health insurance counseling, senior centers, in-home care services, placement services, counseling, and more.

Lawrence County Community Action Partnership is made up of three agencies that seek to involve the community in assessing local needs and attacking the causes and conditions of poverty. They are: Lawrence County Social Services, Inc., United Community Services of Lawrence County, Inc., and Allied Coordinated Transportation Services, Inc. Lawrence County Community Action Partnership reaches out to low-income people to address their multiple needs through a comprehensive approach, develops partnerships with other community organizations, involves clients in agency's operations, and administers a full range of coordinated programs designed to have a measurable impact on poverty. Among its many services, Lawrence County Community Action Partnership provides support in the following areas: Education; employment; emergency and community services; individual and family services; housing rehabilitation; senior services; transportation; and youth programs.

The Lebanon County Assistance Office assists individuals with accessing cash assistance, Supplemental Nutrition Assistance Program benefits (food stamps), childcare payments, medical assistance, SSI advocacy (assists individuals with applying for benefits at the Social Security Office),and emergency fuel assistance.

Lehigh County Office Of Aging and Adult Services provides a variety of services to older adults in Lehigh County. The Office of Aging and Adult Services connects individuals to home delivered meals, assessment and care management, nursing home transition, case management services, senior centers (which provide meals, social activities, computer classes, and more), housing services, adult day centers, public health insurance counseling, personal care services, transportation services, and more.

Simplicity Outreach is a Christian community center that provides one-on-one job search and resume assistance, public benefits counseling, emergency food and toiletries, ESL classes, personal and group bible study sessions, and wellness consultations. Simplicity also distributes free bread, clothing, and household items on a first come, first serve basis every Wednesday at 4:00pm.

The Area Agency on Aging provides services and programs to older adults in Luzerne and Wyoming Counties. The AAA offers employment training and placement, nursing services, active adult center activities (including financial literacy and wellness groups, meals, and social activities), transportation services, caregiver support, volunteer opportunities, health insurance counseling, and more. Please note that these services are offered in multiple locations throughout Luzerne and Wyoming Counties.

The Mifflin-Juniata Area Agency on Aging Inc. (MJAAA) will foster, promote and provide for the development of a comprehensive coordinated service system for persons over the age of 60, within the Counties of Mifflin and Juniata, that enable aging with independence, dignity and respect.

The Monroe County Area Agency on Aging assists older adults in Monroe County with accessing senior centers, home-delivered meals, protective services, personal care services, care management, health insurance counseling, and transportation.

In His Eyes Outreach is an organization that works with previously incarcerated women. Using a network of resources, mentors work to guide women to proper services that will address their individual needs, such as substance abuse classes or treatment, GED and continuing education classes, housing resources, and skills training. Direct services offered by In His Eyes Outreach include: career training, paperwork assistance, recovery support groups, education planning, life skills training, parenting classes, and housing assistance.

The Montgomery County Office of Aging and Adult Services (AAS) provides services to older adults in Montgomery County. Montgomery County AAS assists older adults with accessing in-home care, nursing homes, classes at Montgomery County Community College, public health insurance, senior centers, care management services, home delivered meals, transportation services, in-home behavioral health services, and more. Please note that services are offered at multiple locations in Montgomery County.

The Columbia-Montour Aging Office serves older adults in Columbia and Montour Counties. The Columbia-Montour Aging Office provides care-managed services, home health services, nursing home transition services, in-home services, adult day care services, access to senior centers, meals, health insurance counseling, legal assistance, free transportation services, and more. Please note that an individual may have to travel to Columbia County to receive certain services.

The Hispanic Center of Lehigh Valley aims to "improve the lives of Hispanic and other families." The Community Empowerment Program (CEP) for Adults assists individuals with obtaining employment and other services. There is a senior center for adults over 60 years old (which provides meals, the monitoring of chronic diseases, educational workshops, and recreational activities). In addition, the Hispanic Center provides groceries to anyone who resides in the 18014 zip code once per month; proof of residence (for instance, utility bill, lease, or public assistance documents, SSI documentation) is required. There is also a WIC program.

The Northampton Area Agency on Aging connects older adults in Northampton County with a wide range of services. The agency assists older adults with accessing health insurance, adult day care services, transportation, home-delivered meals, nursing home transition services, personal care services, senior centers, in-home care services, emergency energy assistance, income tax assistance, free legal services, and referrals to other services.

The Emergency Shelter Allowance (ESA) helps homeless or near homeless families and individuals to: stop home eviction or foreclosure; find a long-term place to live; and find a short-term place to live. There are several ESA programs around the state, so please call ahead or check online to find the one nearest you.

Area Agencies on Aging (AAAs) are local aging programs that provide information and services on a range of assistance for older adults and those who care for them. By contacting your local agency you get access to critical information including: Available services in your area; Mobility assistance programs; Meal Delivery; Housing; Assistance in gaining access to services; Individual counseling, support groups and caregiver training; Respite care; and many other supplemental services, on a limited basis.

The mission of the Perry County Literacy Council (PCLC) is to provide educational programs for Perry County residents and their families that will enable them to better function as productive, responsible citizens. All of our programs are 100% free; we tailor our instruction to the individual’s needs and deliver it via one-to-one tutoring and small classes. At PCLC, we provide all of our services at no cost to the student. The only required payment is one-half of the fee for the GED® or HiSET® exam; the other half is covered by PCLC’s Scholarship Fund. PCLC offers a variety of job training courses and can put you in contact with potential employers and other career resources that will help lead you to new employment opportunities. In addition to education and employment assistance, PCLC offers a variety of social services on-site or through referral.

BenePhilly offers free one-on-one support to help Philadelphians enroll in benefits. At the BenePhilly Centers, applicants learn about the benefits available to them, as well as critical supports available through community-based organizations. All BenePhilly Centers also provide benefits screenings and referrals for Women, Infants, and Children (WIC), Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI) and Unemployment Insurance (UI). Community Legal Services (CLS) provides free civil legal assistance to low-income Philadelphians. CLS attorneys and other staff provide a full range of legal services, as well as community education and social work. CLS also assists individuals with obtaining identification documents. CLS' legal units include: Aging and disabilities; Employment; Energy; Family advocacy; Homeownership and consumer rights; Housing; Language access project; and Public benefits. The Connection Traning Services mission is to provide economic, housing and social services for the disadvantaged populations of Philadelphia and to enable participants to forge a path towards self-sufficiency and a better quality of life. Connection Training Services Training-To-Work Adult Reentry Program offers vocational training in forklift operations, scaffold erection, PENNDOT highway faller, and OSHA construction safety. Referrals are available to partnering vocational trade schools, offering state-licensed training in the following career fields with possible tuition vouchers or scholarships: air quality management, automotive technician, construction trades, culinary arts, fiber optics and data communications installation, forklift operator, lead paint abatement, Microsoft Office specialist, naval welding apprenticeship, robotic manufacturing technician, storm water run-off management, truck driver license, urban gardening and landscaping, weatherization, and heating, ventilation, and air-conditioning. CTS also offers courses in job preparation with job placement, life skills, computer applications, and personal finance. Additionally, CTS offers an Access to Recovery Program to support those who are recovery from past substance and/or alcohol abuse.

Impact Services provides a range of employment training and housing support, as well as community and economic development. Impact Services aims to assist TANF recipients and returning citizens with finding work and becoming self-sufficient.

This organization provides a broad range of services for senior citizens. Services include: care at home, advocacy, caregiver support, employment assistance, health promotion, home repair, information and referral, legal assistance, home-delivered meals, protective services, senior community centers, transportation, and many more. Special consideration is given to those with the greatest social, economic, and health needs. PCA employs more than 700 people and provides some services directly; others are provided under subcontracts with 200 community organizations and service providers.

Philadelphia Unemployment Project (PUP) has assisted thousands of unemployed and low-income workers in their job searches and in their dealings with tax and mortgage problems, unemployment compensation, welfare, and health care. PUP also provides Commuter Options (PUPCO), which provides affordable transportation to jobs in the suburbs of Philadelphia.

The Pike County Area Agency on Aging connects older adults with a wide range of services and programs, including home-delivered meals, senior centers, health insurance counseling, adult day services, in-home personal care services, tax assistance, and periodic medical clinics (including eyeglass clinics, blood pressure checks, and flu shots).

The Potter County Area Agency on Aging provides support to older adults in Potter County. The Potter County Area Agency on Aging helps to connect older adults with senior centers, health insurance counseling, transportation, in-home care services, home-delivered meals, personal care services, and more.

The Potter County Assistance Office assists individuals in Potter county seek assistance and a range of services (including cash assistance, Supplemental Nutrition Assistance Program benefits (food stamps), help with child care, health care coverage, home heating assistance (LIHEAP), school meals, SelectPlan for Women and long-term living services.

The Potter County Human Services Office provides access to the Pennsylvania Women, Infants, and Children (WIC) Program. The WIC program is committed to improving the health of eligible pregnant women, new mothers and children, providing nutrition education, breastfeeding support, healthy foods, and referrals to health and social programs during the stages of fetal and early childhood development. The Schuylkill Office of Senior Services provides a variety of services and programs to older adults in Schuylkill County. The Office of Senior Services assists older adults with accessing adult day services, care management, senior centers, meal-delivery services, transportation, counseling, home health support, public health insurance counseling, and more.

The Union-Snyder Agency on Aging (AAA) provides services to older adults. The Union-Snyder AAA facilitates access to in-home care services, long-term care assistance, nursing home placements, home-delivered meals, senior centers (which include computer labs, activities, exercise classes, and more), medical insurance counseling, and more.

The mission of the Area Agency on Aging of Somerset County is to provide supports, services and advocacy to older adults living in our communities so that they may do so safely, comfortably and with dignity.

The PA Health Law Project works to ensure everyone has access to health care. Outside of advocating for the right to health care, the Law Project provides free legal services to any low-income resident of Pennsylvania having difficulty accessing publicly funded health care, whose health care has been terminated or reduced, or who have been denied Medicare (or similar health care programming). Additionally, the Law Project helps eligible residents apply for Medicare, responds to issues of denial of service (i.e. for prescriptions), and anyone struggling with their health care costs. The only requirement is income related, otherwise, the program is nondiscriminatory - there is a helpline which can be utilized for base information by anyone, regardless of income. NEPA Community Health Care operates community health centers which provide physical and behavioral health services. Services include primary care, chronic disease management, disease prevention, mental health services, and more. NEPA accepts most major insurance companies, Medicaid, and Medicare. It also offers a sliding scale fee program for individuals who need assistance with paying for services. In addition, NEPA has application counselors to assist individuals with enrolling in health insurance. Please note that NEPA has three clinic locations in Susquehanna County.

The Venango County Area Agency on Aging offers services and programs to older adults in the community. The Area Agency on Aging assists individuals with accessing health insurance counseling, congregate meals, home modification services, free legal services (including assistance with benefits, rights, wills, and taxes), home care and support, senior centers (which provide meals and social groups), personal assistance services, transportation, and more.

The Venango County Medical Assistance (MA or Medicaid) program provides payment for health care services on behalf of individuals with limited income and high medical expenses. Please call or visit the Venango County Assistance Office for more information.

The Venango County Office of Economic Opportunity provides low-income individuals with home weatherization assistance and transportation services.

Experience, Inc. offers a range of in-home and community-based services to help seniors to have a good quality of life and to remain independent as long as possible. Our professional staff is trained to recognize areas of concern and need and to provide appropriate services and referrals for assistance. Experience, Inc. sponsors Warren and Forest Senior Community Centers. Senior Centers offer a variety of programs and services to help senior citizens maintain their independence, vitality, and provide a place for seniors to have the opportunity to meet other seniors through social interaction. Programs include activities, educational classes, counseling and support groups, volunteer opportunities, and wellness programs. Warren and Forest County Senior Centers also offer meals, help with financial and medical paperwork, transportation, and other social services.

The Southwestern Pennsylvania Area Agency on Aging aims to promote the well-being of older adults in Greene, Washington, and Fayette Counties through a wide range of programs and services. The agency assists older adults with accessing transportation, senior centers, care management, health and wellness programs, adult day care, free legal services (including assistance with Supplemental Security Income (SSI), Social Security benefits, public health insurance), and more. Laurel Legal Services provides free civil legal services to low-income individuals and families. Laurel Legal Services provides housing and utility assistance, employment assistance, and assistance with accessing government benefits, and community education on legal issues. Laurel Legal Services has multiple offices.

The Westmoreland County Area Agency on Aging provides services to older adults in Westmoreland County. The Westmoreland County AAA connects older adults with adult day care services, care management services, home health services, home-delivered meals, personal care services, health insurance, free legal services, senior centers (which provide meals, computer classes, social activities, and more), and transportation. Services may be provided at no cost, a sliding fee scale, or subject to some form of cost-sharing.

The Area Agency on Aging provides services and programs to older adults in Luzerne and Wyoming Counties. The AAA offers employment training and placement, nursing services, active adult center activities (including financial literacy and wellness groups, meals, and social activities), transportation services, caregiver support, volunteer opportunities, health insurance counseling, and more. Please note that these services are offered in multiple locations throughout Luzerne and Wyoming Counties.

The Caring Company is an outreach ministry that serves individuals in need in York County. Services include food assistance, utility relief, prescription medication assistance, and referral services.

GEO Reentry provides flexible options such as residential housing and treatment, day reporting programs, and inpatient treatment and training for individuals returning from incarceration.

COMPASS is an online application for Pennsylvanians to apply for health and human services programs. The application includes Health Care benefits such as Medical Assistance, CHIP, and Medicaid for Former Foster Care Youth. The application also includes Home and Community Based Services Referrals such as Early Intervention Services, Intellectual Disability Services, and Autism Services. Additionally, COMPASS includes Home and Community Based Living Services, Cash Assistance, Low Income Home Energy Assistance Program (LIHEAP), SNAP (food stamps), and other benefits.
