Adams County Office for Aging

The Adams County Office for Aging is a private non-profit agency dedicated to providing assistance to and advocacy for people age 60 and above within Adams County. Our mission is to assist people in maintaining independence and dignity within their own homes and communities. We offer a variety of services to support our age 60 and over population including; adult day care, senior centers, Lifeline systems, medical equipment loan, volunteer opportunities, tax preparation assistance, Medicare counseling, transportation, property tax and rent rebate assistance, protective services, legal services, and a variety of health and educational programs. For in-home care, individual can self-refer or a medical professional can refer as well. As for the other services, interested person should just call ahead and let them know they are coming.

APPRISE Medicare Enrollment Consultation

APPRISE, Pennsylvania's State Health Insurance Program (SHIP), provides free expert Medicare counseling to individuals 65 and older. APPRISE was created by the Pennsylvania Department of Aging to advise Pennsylvania residents about the available health insurance benefits so that they can make informed decisions about their healthcare options. APPRISE provides personalized counseling (over the phone, via email, or in person) to participants in order identify their healthcare needs and preferences, and obtain Medicare and supplemental coverage that suits their lifestyle and budget. APPRISE is housed at Wesley Family Services.

Foundation of HOPE

The Foundation of HOPE is an interfaith non-profit with the mission to “Empower incarcerated and released individuals to restore their relationship with their God, rebuild their lives, and reconcile to their communities.” The HOPE Aftercare Program is a pre- and post-release adult reintegration program for inmates and reentrants that seeks to ensure their successful return to the community. HOPE Aftercare provides resources, referrals, and guidance regarding employment, housing, education, food assistance, mental health services, and clothing. The program’s mentoring component matches inmates and reentrants with mentors who can assist them on a one-on-one basis.

Hill House Association

The Hill House offers a wide range of programs and services to help meet the needs of the community. Accelerated Support Services for Older Adults - A program providing supportive services to persons 60 years and older that have been released from jail or prison and need assistance obtaining housing, food, and clothing. First Source Center - Provides comprehensive workforce development services to citizens that lead to meaningful employment and positively impact the economic health of the community. Home-Delivered Meals - A nutrition program that provides daily meals to homebound older adults who are unable to prepare meals for themselves due to physical or medical reasons. EARN - To assist individuals presently receiving assistance from the Department of Public Welfare to secure and maintain gainful employment. Senior Community Services Center - The senior center provides a place for for seniors to come together within their community and socialize with friends and family. The center also provides resources that address vital concerns for seniors wishing to live independently.

Options/Care: Management Services for Independent Living

Options/Care Management, a program offered by the Family Services of Western Pennsylvania, is for individuals 60 years and older who need assistance remaining independent and living in their own homes. Care managers provide services based on a personalized assessment of the participant's psycho-social, environmental, and financial needs. Participants are eligible to receive home-delivered meals, resource counseling and support, monthly visits from a care manager, and more. Participants' families may also receive training and support through the Family Caregiver Support Program.

Family Service Association of Bucks County

Participants are required to be low-income older adults living in independent housing in Bucks County.

The Family Services Association of Bucks County offers Benefits Navigation for Older Adults, free face-to-face assistance with applying for public benefits to improve financial stability. The program helps low-income older adults apply for: SNAP (food stamps); LIHEAP (heating assistance); PACE/PACENet (help lower prescription costs); Extra Help/LIS (also helps lower prescription costs); Medical Savings Benefit (state pay your Medicare Part B premium); Medical Assistance (provide comprehensive, low-cost health care); Property Tax/Rent Rebate (receive a refund of taxes or rent paid in the prior year). Services are free of charge to low-income older adults living in independent housing in Bucks County. To access services, call the Family Services Association of Bucks County to do a preliminary evaluation of benefits eligibility by phone and then can schedule an appointment to meet at the individual's home or nearby location.

Clarion Area Agency on Aging offers a wide range of programs and cooperates with many community agencies to provide a comprehensive network of services. The Farmer’s Market Nutrition Program provides eligible participants with $20 worth of vouchers to purchase fresh fruits and vegetables at farmer’s markets. Additional programs include individual case management; transportation; prime time health classes; community education; home cooked meals; legal assistance; and in-home supportive care and services. Tri-County Community Action

Acceptance in programs is available to those at or below 125% of the federal poverty level. Proof of income must be made with copies of all pay slips, social security payments, child support payments, and any other income source from the thirty day period before application. If an applicant has not made any income in the last thirty days, they can sign a statement saying so.

Tri-County Community Action provides vocational, life skills, and counseling assistance to low-income individuals. A career training program provides vocational training in welding, manufacturing, and other trades, and completion provides a certificate in job readiness. Completion of vocational training will be supplemented by job search assistance with a case manager. Case managers and clients can work together for up to five years or until clients have achieved self-sufficiency. Case managers also lead group life skills classes and employment development workshops (including resume writing, interviewing, and job searches). The Housing & Credit Counseling program provides housing counselors and classes assisting in home buying, credit counseling, financial fitness, and lending basics. Walk-ins are accepted, but calling ahead is preferred. Tri County Community Action has Cumberland County offices in Shippensburg and Enola, and offices in Dauphin and Perry Counties.

The PA Health Law Project works to ensure everyone has access to health care. Outside of advocating for the right to health care, the Law Project provides free legal services to any low-income resident of Pennsylvania having difficulty accessing publicly funded health care, whose health care has been terminated or reduced, or who have been denied Medicare (or similar health care programming). Additionally, the Law Project helps eligible residents apply for Medicare, responds to issues of denial of service (i.e. for prescriptions), and anyone struggling with their health care costs. COMPASS

COMPASS is an online application for Pennsylvanians to apply for health and human services programs. The application includes Health Care benefits such as Medical Assistance, CHIP, and Medicaid for Former Foster Care Youth. The application also includes Home and Community Based Services Referrals such as Early Intervention Services, Intellectual Disability Services, and Autism Services. Additionally, COMPASS includes Home and Community Based Living Services, Cash Assistance, Low Income Home Energy Assistance Program (LIHEAP), SNAP (food stamps), and other benefits.
