Case Management Resources

Accelerated Support Services for Older Adults is located in the Hill House Senior Services Center, and provides case management and support services for formerly incarcerated individuals that 60 years and older. The program offers assistance obtaining housing, food, and clothing, job readiness, counseling, and in-home services, and support at hearings.

Catholic Charities' Allegheny County Outreach offers services and programs to Allegheny County residents. Allegheny County Outreach provides assistance with obtaining utility assistance and meeting other emergency needs, counseling, and referrals to social service providers when appropriate.

Renewal, Inc. delivers comprehensive drug and alcohol treatment using Cognitive Behavioral Therapy. Renewal offers detox, inpatient, and outpatient treatment services as part of their commitment to supporting the whole individual on their path to reentering society. Renewal also provides case management, life skills training, group treatment programs, workforce development, family support, and community service opportunities.

The Program for Offenders, Inc. provides services to individuals with histories of incarceration to assist them in achieving a fresh start in society and give their children opportunities to achieve. Services include in-patient and out-patient drug rehabilitation, residential services for male and female returning citizens, case management services, and employment services to help individuals assimilate into society as permanent, successful citizens and family members.

The Family Counseling Center provides mental health and intellectual disabilities services to individuals and families in Armstrong County. The Center offers family-based mental health services, medication management, therapy, partial hospitalization services, peer support, psychiatric rehabilitation programs, and case management services. The Center accepts Medicaid, Medicare, and private health insurance plans. Please note that there are multiple locations in Armstrong County.

Berks Connections/Pretrial Services (BCPS) provides reentry programming for returning citizens such as workforce development and housing programs. BCPS also provides services for children and loved ones of returning citizens.

New Person Ministries offers men who are returning from incarceration with transitional housing and case management in order to assist with social services, parole, and housing.

Opportunity House provides emergency shelter and services that include counseling, resume writing, job interviewing skills, mental health and drug and alcohol treatment services, transportation to and from medical appointments when needed, money management supports, and case management services. Opportunity House also provides transition housing for individuals who were engaged in shelter services.

Treatment Access and Services Center, Inc. of Berks County (TASC) is a non-profit organization that offers intake, evaluation, and referral services for individuals in Berks County who are in need of drug and/or alcohol treatment services. TASC also provides case management services for individuals with chemical dependence or mental health conditions.

Blair Senior Services is a non-profit corporation that aims to meet the needs of older adults in Blair County. This includes: case management, transportation, daily meals, educational and recreational activities at four different senior centers, abuse and abandonment protective services, and legal advocacy (Power of Attorney and guardianship services, wills and, long-term care complaints).

CenClear provides mental health, psychiatric, and drug and alcohol recovery services through case management, group and individual therapy, behavioral health clinics, and medication assisted treatment for those battling addiction. The main location is in Clearfield County, and they have sites in Blair, Jefferson, Clarion, Centre, and Elk Counties. Trehab - A Community Action Agency provides a variety of services and programs to individuals across multiple counties in Pennsylvania. Services for Bradford County residents include: case management, housing and utility assistance, weatherization services, and a home ownership program.

Bucks County Department of Mental Health provide services throughout the county. Services include: crisis services, outpatient therapy, case management, and resource coordination. Housing services, psychiatric rehabilitation, and vocational and employment services are also available.

The Cambria County Behavioral Health/Intellectual Disabilities Programs' mission is to view a new horizon of hope and recovery, empowering every individual to direct their treatment and rehabilitation in a manner that is responsive to their individual needs and values and which promotes a satisfying and sustainable lifestyle. Services provided include: case management; socialization recreation; respite care; adult day center; life skills training; vocational training and employment assistance; and residential living arrangements for adults who are interested in developing independent lifestyles. Alcohol & Drug Abuse Services, Inc. (ADAS) offers a range of substance use services. ADAS offers outpatient, intensive outpatient, intervention, case management, prevention, and inpatient services. ADAS accepts Blue Cross, Medical Assistance, Medicare, Community Care Behavioral Health, and other major insurance companies. If an individual does not have insurance, costs are based on a sliding scale. Please note that ADAS offers outpatient services at multiple locations, but inpatient services are only offered at one location in McKean County.

Dickinson Center, Inc. provides services for individuals with intellectual disabilities and/or behavioral health conditions. Behavioral health services include case management services, employment support (including resume assistance and job placement), a C.O.P.E. drop-in center (which includes socialization and recreation services for individuals who are currently receiving or previously received mental health services), and outpatient and inpatient programs. Intellectual and Developmental Disabilities services include adult day services, employment assistance, life skills training, and more.

Northern Tier Community Action Corporation provides services to low-income individuals. Services include assistance with accessing public benefits (including SNAP, medical assistance, LIHEAP), housing assistance (including payment of rent and utilities), case management services, food assistance, and referrals.

The Carbon County Area Agency on Aging helps individuals who are 60+ remain active in their communities by providing case management, transportation, volunteer opportunities, legal assistance, public benefit counseling, assistance with public benefit applications, access to senior centers and recreational activities, and in-home services.

The Carbon-Monroe-Pike Mental Health and Developmental Services provides a wide range of services for individuals with mental health conditions and/or developmental disabilities. The organization offers case management, crisis and emergency care, inpatient care, partial hospitalization, outpatient services, assistance with accessing housing, and life skills training to individuals with mental health conditions. The organization will also link individuals with developmental disabilities with service providers across multiple counties. Please note that Carbon-Monroe-Pike Mental Health and Developmental Services has an office in each county.

CenClear provides mental health, psychiatric, and drug and alcohol recovery services through case management, group and individual therapy, behavioral health clinics, and medication assisted treatment for those battling addiction. The Centre County sites are located in Philipsburg and Matilda.

The Centre County Office of Aging provides services for seniors at six locations throughout the county. The services include: health insurance counseling for Medicare recipients, home delivered meals, investigations of abuse, neglect, exploitation or abandonment of older adults, long-term care services, nursing home transition services, and casework services. The Office also provides funding to outside organizations for personal care services, transportation, adult day services, and legal assistance for those who qualify.

Centre Volunteers in Medicine (CVIM) serves and advocates for the medically underserved living or working in Centre County. CVIM provides medical and dental care, case management and necessary medications at no cost to qualified uninsured and under-insured residents of Centre County and those working in Centre County who live in neighboring counties that do not have free clinics or programs to meet their medical needs. To qualify for care with us: *You must be living or working in Centre County for at least 7 days. *You must NOT have insurance for the services for which you are applying (This includes Adult Basic, Access Card/Medicaid, Medicare, Veterans Benefits). *You must meet income guidelines. Residents must have a household income at or below 250% of the Federal Poverty Level.

The Oxford Area Neighborhood Services Center offers a variety of services to residents of southwestern Chester County. Services include an emergency food pantry (which includes household items like personal care items, cleaning supplies, and paper products), case management services, assistance with payment of utilities, rent, or health care expenses, and more. The main location is in Clearfield County, and they have sites in Blair, Jefferson, Clarion, Centre, and Elk Counties. Clarion Area Agency on Aging offers a wide range of programs and cooperates with many community agencies to provide a comprehensive network of services. The Farmer’s Market Nutrition Program provides eligible participants with $20 worth of vouchers to purchase fresh fruits and vegetables at farmer’s markets. Additional programs include individual case management; transportation; prime time health classes; community education; home cooked meals; legal assistance; and in-home supportive care and services. The Agency has four Senior Centers located throughout Clarion County. The centers offer lunch, and a variety of recreational, social and educational opportunities. Participants enjoy programs, trips and entertainment along with varied evening activities. The Lycoming-Clinton Counties for Community Action (STEP) Office of Aging provides assistance to older adults in Lycoming and Clinton Counties. STEP assists older adults with accessing home-delivered meals, senior centers, transportation services, health screenings and education, in-home services, health insurance counseling, legal services, case management services, assistance with transferring to or from a personal care facility, and more. Please note that there is a STEP office in both Lycoming and Clinton counties.

The Columbia Montour Snyder Union (CMSU) Service System provides a variety of mental health services, substance use services, and services for individuals with intellectual disabilities to individuals in Columbia, Montour, Snyder, and Union Counties. Services include inpatient and outpatient substance use services, employment support for individuals with developmental disabilities, partial hospitalization, vocational support for individuals with mental health conditions, crisis services, case management services, and more. Please note that there are multiple offices and each county has at least one office.

Tri-County Community Action provides vocational, life skills, and counseling assistance to low-income individuals. A career training program provides vocational training in welding, manufacturing, and other trades, and completion provides a certificate in job readiness. Completion of vocational training will be supplemented by job search assistance with a case manager. Case managers and clients can work together for up to five years or until clients have achieved self-sufficiency. Case managers also lead group life skills classes and employment development workshops (including resume writing, interviewing, and job searches). The Housing & Credit Counseling program provides housing counselors and classes assisting in home buying, credit counseling, financial fitness, and lending basics. Walk-ins are accepted, but calling ahead is preferred. Tri County Community Action has Cumberland County offices in Shippensburg and Enola, and offices in Dauphin and Perry Counties.

Alder Health Services offers LGBT-Friendly health services in the Harrisburg area. Alder Health Services provides primary care services, behavioral health supports for individuals who are struggling with anxiety, depression, gender identity, affect and infected by HIV/AIDS, general or major life adjustments, trauma, sexuality, etc. Alder Health Services also offers STD/HIV testing and case management and support.

They have locations in Clearfield, Jefferson, Clarion, Centre, and Elk counties.

Please note that individuals in Elk County will have to travel to Cameron County to receive housing assistance, but there are multiple food banks available in Elk County.

Safe Harbor Behavioral Health provides outpatient treatment for adults who have substance use disorders, as well as who have any behavioral health needs, serious mental illness, or trauma and abuse history. Safe Harbor also provides blended case-management in order to support individuals with mental health needs coordinate community-based psychiatric, medical, and social services. Additionally, Safe Harbor provides individual counseling services.

The St. Martin Center is a social service organization that assists families and individuals with immediate need and long-term services. The immediate assistance includes: food through an in-house food pantry; vouchers for clothing items; assistance with rent, mortgage, and utilities; assistance with life-sustaining prescriptions; transportation assistance for out-of-town medical appointments; a bus token for employment purposes; and referrals to other St. Martin programs or to local agencies. Please note that some of the immediate services can only be utilized once a year. The long-term services include: credit and budget counseling and home buying counseling. The St. Martin Center also provides mens clothing for interviews and jobs of all professions on a referral basis.

Experience Inc. (Area Agency on Aging of Warren and Forest Counties) assists older adults with accessing a wide range of programs and services. Experience Inc. facilitates access to meal delivery services, personal care and home support, housing assistance, case management services, senior centers, transportation, educational classes, counseling and support groups, social activities, and more. Services are offered at multiple locations.

Center for Community Action coordinates, partners, and provides services and resources to promote self-sufficiency in our service area. Among its programs are the following: The Emergency Solutions Grant (ESG) which offers rental assistance, security deposits, and/or utility deposits; The Employment, Advancement and Retention Network (EARN) program which provides case management, education (particularly for individuals with limited English proficiency), skills training, work activities, job placement and retention activities; LIHEAP Furnace Repair; Weatherization; Housing Rehabilitation; and Home Repairs.

The Salvation Army is an evangelical part of the universal Christian Church. The progam's message is based on the Bible and every program is rooted in service. Among its principle ministry services, the Huntingdon Worship & Service Center offers rental assistance, food boxes, volunteer opportunities, and case management services that can meet the individual needs of the client.

Their Jefferson County locations are in Punxsutawney and Brookeville.

The Scranton Counseling Center (SCC) provides a range of services for individuals with developmental disabilities and mental health conditions. Services include mental health diagnosis and treatment services, case management services (including resource coordination), counseling and services for individuals with intellectual disabilities, emergency and crisis services, partial hospitalization programming, and more. If an individual does not have health insurance or is unable to pay, SCC offers a sliding scale fee program.

Since 2001 Beginning In the Right Direction, B.I.R.D. Ministries has been a lifeline of support and encouragement to women trapped in the cycle of prison, addiction and abuse. They provide all that is needed while helping women who have been in prison rebuild their lives. Through a network of resources- from social services agencies to faith-based ministries and churches- they are able to provide the women and their families with the support and services they so desperately need.

New Person Ministries offers men who are returning from incarceration with transitional housing and case management in order to assist with social services, parole, and housing.

A collaborative group of over 50 organizations, the Lancaster County Re-Entry Management Organization (RMO) provides a way for the many groups that work with returning citizens in Lancaster County to leverage their resources in a way that provides community alternatives to incarceration. This organization does not directly provide services, but can help one identify resources in the following areas: drug and alcohol evaluation and treatment, foundation skills assessment; emergency/transitional housing, case management, and legal advocacy.

The Spanish American Civic Association (SACA) offers extensive drug and alcohol services (both inpatient and outpatient), counseling services, HIV/AIDS services. At the Tec Centro, individuals are provided with services to remove any barriers that may inhibit career advancement by offering adults education completion, job readiness workshops, employment services, case management, bilingual training, and multiple career pathways. Among these career pathways are: CNA medical receptionist, medical billing, dental assisting, business and hospitality, construction, manufacturing, and general office (clerical). SACA also has a senior center which offers meals, various activities, and case management services (including housing placement and medical management) to individuals over 60. Please note that services are offered in multiple locations.

TLC, a program of Tabor Community Services, helps individuals, couples and families experiencing homelessness to achieve their housing stability and self-sufficiency goals. The TLC program receives referrals from the Lancaster County Re-entry Management Organization (RMO) to help find returning citizens housing. TLC provides short-term, interim housing and case management for singles and couples.

Catholic Charities' Lawrence County Outreach offers services and programs to Lawrence County residents. Lawrence County Outreach provides assistance with obtaining utility assistance and meeting other emergency needs, counseling, senior services, and referrals to social service providers when appropriate.

Lebanon County Community Action Partnership provides an array of services to Lebanon County residents through a variety of programs. Services include child care assistance, transportation, case management, rental assistance, shelter assistance, bridge and transitional housing, shelter housing, information and referral, job readiness, and life skills education.

The South Central Employment Corporation (SEC) provides employment and education services to youth and adults. Eligibility for services depends on an individual's income and work-status; please call for further information. SEC offers objective assessments, case management services, basic skills instruction, GED preparation, job readiness/job search training, classroom skills training, on the job training, job development, job placement and retention/follow-up services.

Lehigh County Office Of Aging and Adult Services provides a variety of services to older adults in Lehigh County. The Office of Aging and Adult Services connects individuals to home delivered meals, assessment and care management, nursing home transition, case management services, senior centers (which provide meals, social activities, computer classes, and more), housing services, adult day centers, public health insurance counseling, personal care services, transportation services, and more.

Victory House aims to provide shelter services and supportive services to men and veterans who are homeless or reentering society. Victory House offers three meals daily, emergency shelter and transitional housing, case management and counseling, addiction prevention assistance, access to a health center, employment assistance, housing placement assistance, life skills education, community giveback participation, and aftercare.

Luzerne-Wyoming Counties Mental Health and Developmental Services offers services for individuals with mental health conditions and intellectual disabilities. Mental health services include administrative management, case management services, employment services, crisis and emergency services, inpatient and outpatient psychiatric and therapy services, family-based services, and transportation services. Developmental services include counseling, housing assistance, education support services, nursing services, supported employment, and transportations services. Please note that services are offered at multiple locations in both Luzerne and Wyoming counties and an individual may have to travel to Wyoming County receive services. Firetree, Ltd. is a non-profit organization that provides drug and alcohol treatment and criminal justice programs to returning citizens. The inpatient rehabilitation plan includes: daily community meetings, goal setting groups, positive affirmations, peer review, various clinical groups to address addiction, and life skills assessments and classes. The outpatient services at Genesis House provides substance abuse assessments and treatments. The reentry programming includes: case management, community-based support services (AA/NA meetings, mental health, vocational and educational, legal, and religious), recreational programming, and life skills curriculum that covers employment preparation, job placement assistance, basic computer skills, practical money skills, and household management.

Action Wellness is a Philadelphia-based organization committed to helping people living with chronic diseases. Its services include: Chronic illness Treatment & Prevention; Free Rapid Health Screenings (Includes HIV Testing); Medical Case Management; Perinatal & Family Services; Primary Medical Care; Supportive Housing Services; Mental & Behavioral Health; Volunteer Services; Educational and Vocational Assistance.

Bebashi provides a continuum of care (free of charge) involving culturally sensitive and competent community education, HIV, STI, and pregnancy counseling and testing services, and medical case management (for HIV+ individuals). Bebashi also provides breast cancer exams, support and transport assistance. Additionally, to assist those living with HIV and their families to have the proper nutrition necessary to maintain their health, the hunger relief program was established to provide food supplies to these consumers and the general public living below the federal poverty guidelines.

Bethesda Project operates 13 sites throughout Center City Philadelphia for individual adults experiencing homelessness. These sites range from emergency shelter to safe haven provisions to supportive housing to independent living spaces. Each guest and resident completes an Individualized Care Plan with a case manager, with the overall goal of obtaining stable housing and increasing independence.

HELP runs homelessness prevention programs, manages emergency and transitional shelters, and builds and operates affordable permanent rental apartments. HELP also runs service programs that focus on employment, health, and wellness in an effort to solve the problems that cause and perpetuate housing instability. Such programs include security guard training school, culinary training, and counseling and case management.

Lutheran Settlement House (LSH) is committed to serving children, adults, and families in Philadelphia. Programs and services include: Bilingual Domestic Violence Program (BDVP), Community Education and Employment Department (CEED), Homeless Services (Jane Addams Place), Senior Services (Senior Center), Medical Advocacy, and Caregivers Reducing Stress (CARES). Lutheran Settlement House also provides adults with literacy, basic education, GED preparation, computer class instruction, and career-readiness training.

This organization works to develop life plans and create pathways to success for clients. Programs include case management, educational workshops, individual and group counseling, and a peer support network. TCRC also helps individuals obtain health insurance and is the parent organization of the 2nd Chance Project, which provides transitional housing for returning citizens. Resource works to empower disadvantaged, multiracial, minority Americans as well as newly-arrived refugees and immigrants from all parts of the world to achieve social, economic, and educational advancement and mobility. The Potter County Drug and Alcohol Program provides a variety of services, including drug and alcohol assessments; prevention education; assistance in accessing drug and alcohol or mental health counseling; and obtaining or coordinating medical treatment. The Case Management Specialist meets with clients Monday-Friday from 8AM-4:30PM. All clients receive a level of care assessment to determine which type of service is appropriate. The program determines funding eligibility prior to authorizing services.

Mental health services include administrative management, case management services, employment services, crisis and emergency services, inpatient and outpatient psychiatric and therapy services, family-based services, and transportation services. Developmental services include counseling, housing assistance, education support services, nursing services, supported employment, and transportations services. Please note that services are offered at multiple locations in both Luzerne and Wyoming counties and an individual may have to travel to Luzerne County receive services.
