The Allegheny Jail Collaborative is a program committed to reducing recidivism by both preparing inmates for reentry while incarcerated and helping them be successful people upon reentry into society. The program provides services for both inmates and returning citizens. Community Care helps individuals find service providers with the goal of improving the health and well-being of the community through the delivery of effective, cost-efficient, and accessible behavioral health services.

The Foundation of HOPE is an interfaith non-profit with the mission to “Empower incarcerated and released individuals to restore their relationship with their God, rebuild their lives, and reconcile to their communities.” The HOPE Aftercare Program is a pre- and post-release adult reintegration program for inmates and reentrants that seeks to ensure their successful return to the community. HOPE Aftercare provides resources, referrals, and guidance regarding employment, housing, education, food assistance, mental health services, and clothing. The program’s mentoring component matches inmates and reentrants with mentors who can assist them on a one-on-one basis.

The Angora Gardens program offers nutrition, arts and crafts, gardening, personal development, yoga, and holistic health courses. All courses are free unless otherwise stated.

North Hills Community Outreach provides services to low-income individuals and families in Western Pennsylvania under four core values: recognizing the love of God for everyone, treating all individuals with dignity, fostering self-sufficiency, and managing resources in a responsible manner. Services include food services, utilities and emergencies services, employment services, emergency funding help, family development, senior services, school supplies and appliance sharing, computer training, transportation and car-owning, legal, college or post-secondary education, support groups a range of issues, and links to outside resources. Options/Care Management, a program offered by the Family Services of Western Pennsylvania, is for individuals 60 years and older who need assistance remaining independent and living in their own homes. Care managers provide services based on a personalized assessment of the participant's psycho-social, environmental, and financial needs. Participants are eligible to receive home-delivered meals, resource counseling and support, monthly visits from a care manager, and more. Participants' families may also receive training and support through the Family Caregiver Support Program.

Recovery United Pittsburgh operates 5 houses in the South Hills of Pittsburgh, separated by gender, that provide people in recovery from alcohol and other drugs a pleasant and supportive living atmosphere. They also offer Intensive Outpatient Treatment and Group and Individual Counseling at their licensed Outpatient Treatment Services Center.

Renewal, Inc. delivers comprehensive drug and alcohol treatment using Cognitive Behavioral Therapy. Renewal offers detox, inpatient, and outpatient treatment services as part of their commitment to supporting the whole individual on their path to reentering society. The Berks Community Health Center (BCHC) provides physical care services, mental health services, dental services, and other specialized health services to patients of all ages. BCHC also assists individuals with accessing health insurance through the marketplace. There is a sliding fee program to assist individuals with covering health care costs and BCHC has full-time Spanish translators on staff. Bradford County Human Services is the name given to the combined county agencies of Child Care Information System, Children and Youth Services, Drug and Alcohol, Mental Health, and Intellectual Disabilities. This office also administers the Human Services Development Fund and Homeless Assistance Program for the County Commissioners. The mission of Highlands Health is to provide free medical and pharmaceutical care to the low income uninsured population in the Laurel Highlands region of Western PA. A person may receive free doctor check-ups, free medicine, free X-rays, free blood tests, and a variety of other basic tests. Naloxone, also called Narcan and Evzio, an easy-to-use prescription medicine to reverse opioid overdose, is available free of charge, no questions asked. To be a patient at the Free Clinic a person cannot have any health insurance. He/she cannot have medical assistance from the Pennsylvania Department of Welfare or be eligible for Medicare. The person must have a photo ID and show proof of total household income. The person will be required to obtain a denial letter from the Medical Assistance Office within 90 days. The person will have an Intake appointment to determine if he/she is eligible for clinic services before seeing the doctor. Additional forms to fill out are available on the website. All appointments must be scheduled in advance.

Alcohol & Drug Abuse Services, Inc. (ADAS) offers a range of substance use services. ADAS offers outpatient, intensive outpatient, intervention, case management, prevention, and inpatient services. ADAS accepts Blue Cross, Medical Assistance, Medicare, Community Care Behavioral Health, and other major insurance companies. If an individual does not have insurance, costs are based on a sliding scale. Please note that ADAS offers outpatient services at multiple locations, but inpatient services are only offered at one location in McKean County.

Dickinson Center, Inc. provides services for individuals with intellectual disabilities and/or behavioral health conditions. Behavioral health services include case management services, employment support (including resume assistance and job placement), a C.O.P.E. drop-in center (which includes socialization and recreation services for individuals who are currently receiving or previously received mental health services), and outpatient and inpatient programs. Community Volunteers in Medicine (CVIM) offers free coordinated health care. CVIM offers physical and dental services, as well as referrals to a network of local physicians, hospitals, and other healthcare providers to offer comprehensive care.

Community Youth and Women's Alliance (CYWA) offers a temporary shelter for homeless women and children, but also make special accommodations for homeless men raising their children as well as intact families. CYWA also provides safe, clean, and affordable housing to low income individuals and families; with the low rent program creating greater independence. Additionally, CYWA offers weekly individual therapy, as well as coed groups, women's groups, men's groups, and specialized groups. CYWA also transports adults to Human Services, Inc for any apparent mental health issues.

Devereux Advanced Behavioral Health provides intellectual and developmental disability services (treatment in residential settings, community-based living, outpatient services like medication management and family counseling, special education day schools, employment and transition services) and mental health assessments and treatment (for anxiety disorders, bipolar disorder, post-traumatic stress disorder, depression, ADHD, psychotic disorders and disruptive behavior disorders) for adults in Wayne and Chester County. Their principle Pennsylvania location is in Delaware County.

La Comunidad Hispana offers individuals supports such as physical exams, wellness visits, behavioral health counseling, testing and treatment for treatment of sexually transmitted diseases, dental care, and referrals to specialty care. La Comunidad Hispana also offers dental services, free ESL classes, GED classes, citizenship classes, and job placement services. RASE of Carlisle serves those recovering from addiction by offering services tailored to the person's needs and interests. Services include career training workshops, recovery support services, one-on-one recovery coaching, buprenorphine support, group meetings (education life skills, vivitrol, educational support group, recovery 101), and recovery coaching. Sadler Health Center specializes in providing services to the uninsured, underinsured, Medicare and Medicaid eligible individuals. Sadler provides Medical, Dental, Behavioral Health and Tobacco Cessation as well as Prescriptive Services and the Nurse Family Partnership program. Sadler Health Center is staffed with a team of qualified professionals including Family Physicians, Nurse Practitioners, Licensed Clinical Social Workers, Dentists and Dental Hygienists. Alder Health Services offers LGBT-Friendly health services in the Harrisburg area. Alder Health Services provides primary care services, behavioral health supports for individuals who are struggling with anxiety, depression, gender identity, affect and infected by HIV/AIDS, general or major life adjustments, trauma, sexuality, etc. Alder Health Services also offers STD/HIV testing and case management and support.

CRESC is a network of caring professionals and community leaders whose purpose is to provide encouragement, hope and resources for the formerly incarcerated individuals. CRESC provides a variety of services, including mentoring, housing, employment, and referrals to other resources.

CRAM, Inc. is a faith-based organization that provides pre-release, reentry, and ongoing support services to currently and formerly incarcerated individuals of any faith or background. CRAM, Inc. offers mentoring, substance use, GED preparation, computer and financial literacy, family reunification, and employment services. They also provide HIV/AIDS services, community service opportunities, and assistance with transportation.

GEO Reentry provides flexible options such as residential housing and treatment, day reporting programs, and inpatient treatment and training for individuals returning from incarceration.
